• Undergraduate and Graduate: Submit certified transcripts for all colleges/universities attended, including the latest Fall semester/Winter quarter.

Transcripts must remain sealed to be considered official.

List your post high school employment by date, citing any relevance to your studies. 

7. AUTOBIOGRAPHY, COMMUNITY SERVICE, and CAREER OBJECTIVE   
Submit a typed, double-spaced statement of no more than two pages explaining the connection between your career objectives, family background and track record of community activities and concerns. Explain briefly how you expect your college education to help you achieve your goals. If you have no community service activities, indicate how you plan to contribute to the community in the future. (Be specific and as concrete as possible.)
